These documents (signed professional practice report form and Internship tasks form also) must be included into the practice report (as a supplement) - at the end of the report.  

N.B.: If a student doesn’t submit one of those documents to academic consultant, he is not allowed to defend his practice report.

Professional practice report

Placement report with requested supplements, signed by student and both coordinators (from the VU BS and the company), must be submitted to BS supervisors one week after the internship ends.

N.B.: Students, who are not submitting documents on time or any of the requested supplements are missing, won’t be allowed to defend their placement reports.
